Donation Request Form
Thank you for thinking of us for help with your fundraising efforts! There are many worthwhile organizations supporting the community that Whole Foods Market would be proud to partner with.
If you would like to submit an application for a donation, please provide the necessary information–Incomplete applications will be denied.
If we can accommodate your request, you will be contacted. Due to the volume of requests, please do not contact the store to inquire about the status of your application.
For all donation requests, please fill out our Donation Application and return via email, mail, or drop off at the front desk. Please send donation requests and any inquiries to sadie.uthe@wholefoods.com. Community Giving Day and Bag Donation Program applications are available at our front desk.
